Support Coordination

At The iCare Group Australia, our Support Coordination services are designed to help participants navigate the NDIS, connect with the right supports, and build the skills and confidence needed to manage their supports more independently over time.

We understand that the NDIS can often feel complex and overwhelming for participants, families, and carers. Our experienced Support Coordinators work alongside participants to simplify the process, provide guidance, advocate where required, and help ensure supports are practical, effective, and aligned with individual goals.

We understand that every participant’s journey is different.

Some people may need assistance navigating complex systems, while others may require support during periods of transition, crisis, housing instability, hospital admissions, or significant life changes.

Our team has experience supporting participants with psychosocial disabilities, complex care needs, behavioural supports, housing challenges, and high intensity support requirements. We work collaboratively with participants, families, guardians, hospitals, allied health teams, mental health services, and providers to help ensure supports remain coordinated, responsive, and participant focused.

At The iCare Group Australia, we also recognise the importance of strong provider relationships and effective communication. We focus on building collaborative support networks around participants to help reduce gaps in care, improve consistency, and support better long term outcomes.

Recovery Coching

At The iCare Group Australia, our Psychosocial Recovery Coaching supports are designed to help participants living with psychosocial disabilities build capacity, strengthen independence, and work towards a more meaningful and connected life.

We understand that recovery looks different for everyone. Recovery is not about rushing progress or expecting people to fit into unrealistic expectations. It is about working alongside participants to build confidence, stability, resilience, and practical strategies that support long term wellbeing and independence.

Our Recovery Coaches provide person centred support that focuses on understanding the participant as a whole person, not just their diagnosis or challenges. We work collaboratively with participants, families, carers, mental health services, support networks, and providers to help create supports that are practical, sustainable, and aligned with individual goals.

Recovery Coaching may support participants with:

  • Building independence and daily living skills
  • Developing routines and structure
  • Improving confidence and motivation
  • Strengthening social and community connections
  • Navigating mental health and support systems
  • Coordinating supports and services

Building self advocacy and decision making skills

Our Recovery Coaches provide flexible and compassionate support tailored to each participant’s unique circumstances and recovery journey.

We focus on building genuine relationships and creating safe, supportive environments where participants feel heard, respected, and empowered. Our approach is strengths based, trauma informed, and focused on helping participants identify what matters most to them.

Our Recovery Coaches also work collaboratively with support coordinators, support workers, allied health professionals, hospitals, mental health teams, and families to help ensure supports remain consistent, coordinated, and responsive as needs change over time.

With strong operational systems and access to broader multidisciplinary supports including housing, nursing, community participation, and daily living supports, we are able to provide holistic and flexible support arrangements tailored to a wide range of psychosocial support needs.
