Support Coordination & Recovery Coaching

Support Coordination

At The iCare Group Australia, our Support Coordination services are designed to help participants navigate the NDIS, connect with the right supports, and build the skills and confidence needed to manage their supports more independently over time. We understand that the NDIS can often feel complex and overwhelming for participants, families, and carers. Our experienced Support Coordinators work alongside participants to simplify the process, provide guidance, advocate where required, and help ensure supports are practical, effective, and aligned with individual goals. At The iCare Group Australia, we take a person centred approach to Support Coordination. We take the time to understand each participant’s circumstances, support needs, goals, risks, and long term vision so we can help create supports that genuinely improve quality of life.
We understand that every participant’s journey is different.
Some people may need assistance navigating complex systems, while others may require support during periods of transition, crisis, housing instability, hospital admissions, or significant life changes.
Our team has experience supporting participants with psychosocial disabilities, complex care needs, behavioural supports, housing challenges, and high intensity support requirements. We work collaboratively with participants, families, guardians, hospitals, allied health teams, mental health services, and providers to help ensure supports remain coordinated, responsive, and participant focused.
At The iCare Group Australia, we also recognise the importance of strong provider relationships and effective communication. We focus on building collaborative support networks around participants to help reduce gaps in care, improve consistency, and support better long term outcomes.
Our Support Coordinators aim to empower participants by helping them understand their options, make informed decisions, increase independence, and build confidence in managing their supports and achieving their goals.
With strong operational systems, experienced leadership, and access to multidisciplinary supports including nursing, housing, psychosocial, and community services, we are able to support participants across a wide range of circumstances and support needs.
At The iCare Group Australia, we are committed to providing Support Coordination that is proactive, compassionate, responsive, and focused on creating meaningful outcomes for participants and their families.

Recovery Coaching

At The iCare Group Australia, our Psychosocial Recovery Coaching supports are designed to help participants living with psychosocial disabilities build capacity, strengthen independence, and work towards a more meaningful and connected life.
We understand that recovery looks different for everyone. Recovery is not about rushing progress or expecting people to fit into unrealistic expectations. It is about working alongside participants to build confidence, stability, resilience, and practical strategies that support long term wellbeing and independence.
Our Recovery Coaches provide person centred support that focuses on understanding the participant as a whole person, not just their diagnosis or challenges. We work collaboratively with participants, families, carers, mental health services, support networks, and providers to help create supports that are practical, sustainable, and aligned with individual goals.

Recovery Coaching may support participants with:
